Dual Enrollment Advisor: Michele Menadue
Step 1: Print the Homeschool Checklist
Track your progress using the “Homeschool Checklist” . Check each box as you complete a
step to be sure all requirements are met. Incomplete applications will not be processed.
Step 2: Complete the Online Dual Enrollment Application
Complete the SJR State Dual Enrollment Application online.
- Select “Dual Enrollment – Private or Home School” as the Admit Type.
- Do not select the county you live in.
- A parent must sign the Student/Parent Consent Form.
- The Counselor Consent Form will be emailed to your parent to sign as the high school “counselor.”
Once approved by Admissions, you will receive your student ID number at the email address used in the application.
Step 3: Provide Placement Test Scores
Parents must upload ACT, SAT, PERT, PSAT, CLT, or accepted placement test scores in the Counselor Recommendation/Consent Form. Parents: Do not complete the counselor consent page until you have the test scores readily available to upload/attach.
Only upload a copy of test scores, no other documents. If you do not have test scores, schedule the PERT with the SJR State Testing Center. Scores determine placement in English and Math; no minimum score is required for admission. Visit the
Testing Center webpage for more info and PERT payment.
Step 4: Counselor Consent Form
The Counselor Consent Form must be emailed to the parent (as the homeschool counselor).
- Only upload test scores.
- Do not upload additional documents.
Step 5: Verification Form
Complete the top portion of the Homeschool Verification form. The county will complete the bottom portion. After it’s filled out, submit it in the final packet (Step 7).

Personalized Education Program (PEP)
If you receive PEP funding, you will not be registered with the county. Complete both sections of the Verification Form with your award information and include it in the final packet (Step 7). Do not send it to the county.
Step 6: Sign the Homeschool Articulation Agreement
Review and sign the current year’s agreement. Include the signed signature page in the final packet (Step 7).
Step 7: Submit the Final Admissions Packet
Your packet must include:
- Signed Homeschool Checklist
- County Verification Form or PEP Verification Form
- Signed Articulation Agreement signature page (student + parent)

Step 8: Zoom Meeting
After your packet is approved, register for a Zoom meeting with Michele Menadue. A scheduling link will be provided.
Step 9: Dual Enrollment Registration Form
After your Zoom meeting, complete the Registration Form using your X number. Ensure a parent signs the “high school counselor” signature line. Email the form to for final review. Processing time may delay schedule updates.
Step 10: MySJRstate Account
Set up your MySJRstate account at SJRstate.edu.
Allow up to 2 weeks after the Counselor Recommendation/Consent Form for your account to activate. Contact Michele Menadue at 386-312-4285 if needed.
Step 11: Online Orientation
Once your account is active, log in to Canvas through MySJRstate and complete the required New Student Orientation under “Courses.”
Step 12: Verify Your Schedule
In MySJRstate,
go to the Student tab and select Concise Student Schedule. Email any errors to and include your X number plus a brief description.
Step 13: Textbooks
Bring a printed schedule to the SJR State Bookstore to pick up textbooks.
- You must sign a rental agreement.
- Books are available only after attending at least one class.
- Any book not returned at semester’s end will be charged at full price
Step 14: Accommodations (if needed)
If accommodations are needed, visit any Advising Center and meet with the Disabilities Coordinator. See Disability Services for details.
